A. It's done by software at the exchange the stocks are listed on. Each stock is quoted on a particular exchange, in the UK that's London Stock Exchange (LSE), AIM (Alternative Investment Market) or Offex (stands for Off Exchange - I think it may have a new name but I can't recall it). Not counting any off-market trades, most dealing will be done on the exchange. The highest and lowest bid and offer prices will be calculated each day based on the trades and the share price is then an average somewhere in the middle of those trade prices. Look up a stock on londonstockexchange.co.uk to see an example.
